The data requested in this form is required for galactic contact.

Please provide all of the required information in one return email, with your
pictures as attachments.  Incomplete forms cannot be entered into our master spreadsheet.
When our galactic friends are ready to implement our ‘Meet ‘n Greet’s’, your data will be transferred to them by one of our advanced telepaths.

Some Tips!!
The problem area for most participants seems to be in providing the coordinates for their landing
site.  This is a very critical piece of data, and is easier to do than you may think.
When choosing your site, try to have something that would stand out in a picture of the area.  A park, a major road, a lake, or a large building.  This will be of great assistance to you, when trying to pinpoint your location on Google Earth to get the coordinates.
Go to the Google search engine, and type in Google Earth…this is a very cool piece of technology, that brings up the whole world.  Simply type your site location into the ‘fly to’ area by entering the name of the closest town, city or landmark to the actual site… and you will be close to your landing spot.  Use the yellow tack cursor and move the earth in closer, or rotate it; whatever is needed to get the tack positioned exactly on your landing site.
When you have the tack correctly positioned, note the coordinates, which will appear on the screen, most often in the lower left corner…this is the latitude and longitude of your site.
The second required picture is a personal photograph of the site host…you.  Your picture should accompany your data information as an attachment when you return your form via email.
